OPTOMETRIST OF THE YEAR

Eligibility Criteria:

1. The nominee must be a MOA member for a minimum of five (5) years.
2. The nominee must be nominated by an individual MOA member or an optometric society.

Procedure:

1. Nominations must be submitted online.
2. Supplemental information (letters of recommendation, curricula vitae) may be uploaded.
3. Nomination of a nominee in any given year does not preclude future nominations of the same nominee.
4. Completed nomination forms must be received by the MOA by September 1st.
5. The MOA Optometrist of the Year Award will be presented at the annual MOA convention.
6. If a suitable nominee is not selected by the judging committee, no award will be given that year.
